runtime_policy.secret_binding_environment_mismatch
Bound secret ID exists in the organization but belongs to a different Environment than the policy. `400` (not `404`) is a deliberate intra-tenant carve-out per ADR-0062: the configurer is already scope-authorized, so distinguishing wrong-env from not-found is not an existence oracle.
